If your chiropractic or acupuncture medical claims have been denied for unknown reasons or approved but remain unpaid, we need your help to get them corrected and released. Only your voice as a member can make a difference in this matter. Please contact the appropriate local office, your Health and Benefits Office, or the Health & Welfare Office to request the release of these unpaid claims. Your action ensures you receive the care you deserve without unnecessary delays.
Below are the names our office sends medical claims for services we render:
